Our practice specializes in Gender Affirming Care. We believe that all patients deserve accessible, equitable and competent care. We work with our patients to create a personalized transition profile based on individual goals and current best practices. No two patients are identical and each person deserves to play an active role in achieving their transition goals. Given the shortage of providers that specialize in Gender Affirming Care, we have elected to offer services for both members of the DPC practice as well as non-members. In addition to the many other benefits outlined on the website, the Gender Affirming Care Plan also includes: Guaranteed follow-up visits every 3 months As-needed telemedicine visits if any issues/ questions arise Letters for name and/or gender marker changes Medical letter of support for surgery Discounted price on additional services offered in clinic
