I need an initial evaluation - how do I set this up?

Accepted Answer

If you require an initial evaluation you will register for the service you are interested in and when you are scheduling your first office visit you will make a note that an initial evaluation is needed.

This will prompt the system to charge the initial evaluation fee. Given that you have already paid your monthly membership, you will not receive a bill for your second month.

Question 2

I am fairly healthy and don't require frequent medical visits, should I still join?

Accepted Answer

Yes!  Prevention should be the cornerstone of Family Medicine. Most patients' wait to get medical care until they absolutely need it, leading to emergency's and other negative health impacts.

At Outpatient Family Medicine Transformed our goal is to be proactive and work with you to keep you healthy by giving you access to same or next day appointments, as well as direct access to texts or phone calls with Dr. Lapinski to answer any questions you may have.

Question 3

If I already had an initial evaluation for gender affirming care, will I need to be re-evaluated?

Accepted Answer

No! If you already had your initial evaluation you will not need a re-evaluation and will automatically start with the "Follow-Up" monthly plan.

What if I decide to cancel my membership?

Accepted Answer

We understand that things change and our practice may no longer fit your needs.

If you need to cancel, we ask that you give us a 30-day notice.

This will allow us to close out your account and help you with transitioning your care.
